Please use the following form whenever convenient in place of phoning or coming in person to the City Secretary's Office. We intend to minimize your time, expense and effort, as well as automatically document and track your request.

Our goal is to process your service request within five to ten business days of receipt. You will be contacted if additional time is required. Certifications require a minimum of five day, and up to ten days to complete.
